Flu Shot Clinic

The City of Basehor in partnership with Healthy Solutions, Inc. is hosting a 2-day Flu Shot Clinic at Basehor City Hall.

Please complete, print, and sign the consent form (located at the bottom of the page) and bring it to the clinic if you want to receive flu shot. If possible, please make a copy of your insurance card (front & back) and attach it to the consent form. Remember to bring your health insurance card the day of the clinic. 

Please call Basehor City Hall at 913-724-1370 if you want to participate in the flu clinic. RSVP’ing will assist Healthy Solutions, Inc. with proper staffing needs and ensure the correct amount of vaccines are on hand.
